To make a payment and review payment status click the Pay Now button on the home page or click here. Check payments can be mailed to: Cox Health Plan, P.O. Box 74008496, Chicago, IL 60674-8496. For Overnight Mail, the address is: Bank of America Lockbox Services, Cox Health Plan 8496, 540 W. Madison, 4th Floor, Chicago, IL 60661. Cash payments can be made at Walmart. A copy of your most recent invoice is required to make a cash payment. The phone number to check payments is (866) 591-4269.
